Niagara on the Lake Residents requesting PTS forms for AAA fall 2022 Tryouts must email OMHA Rep Toby Gula

The following information MUST be included in the email request or a PTS will NOT be issued. First Name , Last Name , Residential Address , Date of Birth , How long has the player lived at the current Niagara Address , Team and Division played last season , what Association was the player enrolled with last season , What team and level are you trying out for , contact name , phone number and email address. NRP Passport Requests are for U12 , 13 , 18 ONLY .With the NRP you can ONLY tryout for one team
EITHER A or AA. If you request AA and tryout and you DO NOT make it.
You CANNOT come back and request another one to try out for A. Only ONE NRP per player per year, these are OMHA rules. Once the PTS is issued via email you will be required to PRINT A PAPER COPY AND BRING TO THE TRYOUTS. NRP forms for AA and A must be requested and picked up from TOBY GULA directly , refer to each association for tryout schedule , fees and requirements.
